Community Habilitation Services

A valuable skill training program where a worker of your choice will coach your family member new skills and supports to gain more independence, socialization and activities of Daily Living skills.

Rayim’s staff members together with your service coordinator will, with your involvement, review a consumer's strengths and weaknesses and assess the areas where support is most needed.  They will create a specialized plan on how to obtain these goals. Rayim's staff communicates with individuals, employees and parents to assure that each individual reaches their utmost potential.

An essential part of all of our consumer's Com Hab budget is the OTPS and Clinical Fund. 

• The OTPS fund offers a quarterly reimbursement of $250 towards items bought to acquire or enhance any Com Hab goal mentioned in the individual's plan. 
• The Clinical fund offers a quarterly reimbursement of $250 towards training or consulting the individual's Com Hab worker.  This training marks a frame of structure for the worker and ensures quality service for the consumer.

If your family member has been approved through Rayim for the Community Hab service, you may submit the following voucher along with an itemized receipt for the amount to be reimbursed.
